Well, we have to remember that the poop shoot is designed to eliminate waste. Thankfully, most of shit, is, well, shit!! But mixed along with these undigestible waste products can be blood (from hemorrhoids, cracks or fissures in the anal canal, bleeding from higher up in the GI tract (anywhere from the duodenum of the stomach on down!), bacteria and viruses.
That's the rub. Some of these germs (bacteria, parasites, amoeba, viruses) can potentially spread something to you. Hepatitis A, for example, can be easily spread from the ass (fecal contamination) of an infected person to the mouth and gastrointestinal tract to an uninfected person, unless of course, you've already been vaccinated against hepatitis A, which is highly recommended. As can herpes, and syphilis, and giardia & amebiasis. Infectious bacteria like campylobacter, shigella, and salmonella, can also be spread through rimming or other mouth to anal contact. Unfortunately, no preventive vaccinations for these nasty germs. And no amount of douching, enemas, and scrubbing can prevent these germs from getting spread, if they are there. Douching and enemas and scrubbing can sometimes make it easier to get these infections. But the germ has to be present in order for it to be potentially spread.
HIV is not quite as straight forward. The virus must make a direct line to the uninfected person's blood supply through a fresh cut in the mouth; HIV is not effectively transmitted in this manner.
